# Clinical documents

> Create and manage clinical notes, referrals, and related documents.

# Clinical documents

Requires **Clinical documents** permission.

Use **Clinical documents** for notes, discharge summaries, referrals, and similar records linked to patients.

Search matches titles (and note text). Closer title matches appear first when you type.

## Create a document

1. Open **Clinical documents**.
2. Choose **New** (or equivalent).
3. Select the **patient** and document type.
4. Write content and set status (**Draft** or **Final** as your process requires).
5. Save. The document appears in the list and on the [patient chart](/patients/chart).

## Review and update

Open a row to use the detail drawer. Edit drafts, finalize when ready, and attach files when your workspace has storage configured.

Closing create/edit with unsaved changes (or refreshing the page) asks you to confirm before discarding.

## Tips

* Finalized documents may be restricted from casual edits — follow your clinic policy.
