# Lab reports

> Draft, sign off, publish, and amend laboratory reports.

# Lab reports

Requires **Lab management** permission.

## Prerequisites

* An active [lab](/lab/labs)
* A populated [catalog](/lab/catalog)

## Find a report

On **Laboratory → Reports**, use search (report id, patient name, or lab name). Better matches appear first while you type.

## Create a report

1. Open **Laboratory → Reports**.
2. Start a **new report**.
3. Choose the **lab**, patient mode (**linked** patient or **standalone**), and **referred by** when needed.
4. Enter results against catalog tests. Use internal notes for staff-only comments.
5. Save as **draft**, or submit for **pending sign-off** when ready.

Closing a draft editor or refreshing the browser with unsaved draft changes asks you to confirm before discarding.

## Publish workflow

| Status           | Meaning                   |
| ---------------- | ------------------------- |
| Draft            | Editable; not final       |
| Pending sign-off | Waiting for an approver   |
| Published        | Final for print and chart |

Approvers can **approve** (publish) or **return to draft**. Overview may list pending sign-off items for people who can approve.

## Amend

Published reports can be returned to draft or amended according to your clinic process. Re-print after changes when paper copies must match the system.

## Patient chart

Linked reports appear on the [patient chart](/patients/chart). Standalone reports stay on the reports list with the identifiers you entered.

## Print

Use **Print** on a published report. Configure layout and QR under [Lab printing](/lab/printing).
