# Patient chart

> View demographics, visits, labs, and documents on one patient.

# Patient chart

Requires **Patient records** permission (and related module permissions for linked sections).

Open a patient from **Patient records** to see their chart.

## What you see

Typical chart sections (visibility depends on permissions and data):

* **Demographics** — identity, contacts, photo
* **Appointments** — visits linked to this patient
* **Lab reports** — laboratory results for this patient
* **Clinical documents** — notes and referrals
* **Print chart** — print-friendly summary when available

## Link work to the chart

When you book an appointment, create a lab report, or add a clinical document, you can link it to this patient so everything stays on one chart.

## Archived in this workspace

If the patient was removed from the workspace, the chart shows an **archived in this workspace** banner. Choose **Restore patient** (when you have update permission) to mark the membership active again. History such as lab reports remains on the chart.

## Tips

* Use search from **Patient records** if you opened the wrong chart.
* Dates and times use your [workspace timezone](/admin/workspace-settings).
