# Patient records

> Search, register, and manage patients in ClinZero HMS.

# Patient records

Requires **Patient records** permission.

Use **Patient records** to find existing patients and register new ones for your workspace.

## Find a patient

1. Open **Patient records** in the sidebar.
2. Use search and filters (name, identifiers, status) to narrow the list. Closer matches appear first when you type a search term.
3. Click a row to open the [patient chart](/patients/chart).

## Register a patient

1. Choose **Add patient** (or equivalent primary action).
2. Enter demographics and required fields (including consent fields your workspace collects).
3. Save. The new patient appears in the list and can be opened as a chart.

  Only enter real clinical data in the live system. Do not paste unrelated personal data into demos or screenshots.

## Edit a patient

From the list or chart, open edit to update demographics, photo, and contact details your role allows. Changes apply only inside the current workspace.

If you close the editor or refresh the page with unsaved edits, ClinZero asks you to confirm before discarding them.

## Remove from workspace

Requires **Patient records** delete permission.

1. On **Patient records**, open the row actions and choose **Remove** (confirm **Remove patient from workspace?**).
2. The patient is marked **inactive in this workspace only**. Existing lab reports and chart history stay; they are not deleted.
3. If the same person were linked in another workspace, that membership is unchanged.

Inactive patients are hidden from default lookups. Filter the list by status to find them again.

## Restore

From an inactive patient’s [chart](/patients/chart), choose **Restore patient** (requires update permission). That marks the membership active again in this workspace.
